How can we help?
Where can I find or request a form?
Forms & Downloads brings together the available official KGMB forms. The membership form can be prepared using the form assistant; the two investment forms are available as fillable PDFs. Print and sign completed forms before returning them.
If the form you need is not listed, request it through General Enquiry / Feedback.
Where can I access my member account?
Use the Coop-Care Member Login button to open your member portal. Your member account is separate from this public website.

Read the eligibility and contribution information on the Membership page. Prepare the official membership form, print and sign both pages, and include the required supporting documents. Use New Membership Submission on Contact Us to send your signed PDF. Membership is subject to management approval.

How do I send a signed form?
Print and sign the completed form, then scan the signed pages into one PDF of up to 5 MB. On Contact Us, choose the relevant category and attach your PDF. Follow the form’s instructions for supporting documents.
If online submission is unavailable, email the relevant department directly. Sending a form does not confirm approval; KGMB will review your application or request.
Where can I find investment rates?
The Savings & Investment Schemes page shows the published rates, their effective date and a comparison calculator. Choose an amount, tenure and payment schedule to compare the two schemes.
